What Should I Bring To My Visit?

  • Current Insurance Card or Social Security Card
  • Proof of Income such as your Paycheck Stub, W2 Form, Letter from Employer
  • Current ID such as Driver's License, Green Card, or Passport
  • For Physical Exams (Insurance, Work or School), All Associated Paperwork which needs to be filled out by our Health Care Provider
  • Proof of Address such as your Driver License, Household Bills, Rental Agreement
  • For Prenatal visits: Proof of Pregnancy if Pregnancy Test has not been done with MCHC

What If I Have No Insurance? What Are My Options?

Patients who do not have health insurance may be seen as self-pay patients through our sliding fee scale. Self-pay patients will be charged for services based on their income and family size. Patients should bring proof of income to their first appointment. Most recent tax return, last pay stub or letter from employer is fine.

There are many programs available through MCHC to assist self-pay patients and those with high insurance deductibles to obtain affordable health care. Please ask an MCHC staff member for more information.
